Prime Allied Workforce is committed to maintaining a safe, respectful, ethical, and compliant environment for our employees, candidates, clients, partners, contractors, and the communities we serve. If you become aware of an incident involving safety, workplace conduct, operations, transportation, data security, discrimination, harassment, or another matter requiring review, please report it promptly so the appropriate PAW team can assess and respond.

What Should Be Reported?
Use the Incident Reporting process when an event has occurred that may require documentation, investigation, corrective action, or follow-up.

Workplace Safety
Injuries, unsafe conditions, hazards, property damage, equipment-related incidents, or near misses.

Transportation & Vehicle Incidents
Vehicle accidents, passenger incidents, driver-related concerns, fleet damage, transportation safety events, or operational disruptions.

Workplace Conduct
Harassment, threats, inappropriate behavior, workplace conflicts, retaliation concerns, or violations of professional conduct expectations.

Client or Service Incidents
Incidents occurring at a client location, service disruption, client complaint, staffing incident, or event affecting service delivery.

Information & Cybersecurity
Suspected unauthorized access, lost devices, phishing, privacy incidents, data exposure, or cybersecurity concerns.

Other Reportable Events
Any significant event that may create a safety, operational, legal, reputational, or compliance concern for Prime Allied Workforce.

Do not use this website form for emergencies or situations requiring immediate police, fire, or medical assistance.
Call 911 or contact the appropriate emergency authority first.
After the immediate danger has been addressed, submit an incident report to Prime Allied Workforce as soon as reasonably possible.
Is This an Emergency?
Before You Submit a Report
Please provide as much accurate information as reasonably available. You do not need to know every detail before submitting a report.
Helpful information may include:
• Date and time of the incident
• Location
• Individuals involved
• Description of what occurred
• Witnesses
• Injuries or property damage
• Actions already taken
• Photographs, screenshots, documents, or other supporting information
Do not delay reporting because documentation is incomplete. Additional information may be provided during follow-up.
Submit an Incident Report
Use the secure PAW Incident Reporting Form to provide information about the event. Reports are routed for review based on the nature of the incident and may be reviewed by Risk & Compliance, Human Resources, Transportation & Logistics, Information Technology, executive leadership, or another appropriate PAW function.
Please submit one report per incident. Do not submit multiple reports for the same event unless specifically requested by PAW.
What Happens Next

Report Received
Your submission is documented and routed to the appropriate PAW function.

Initial Review
The report is reviewed to determine urgency, risk level, required notifications, and next steps.

Follow-Up & Investigation
PAW may request additional information, interview relevant individuals, review documentation, or initiate an investigation.

Resolution & Corrective Action
Appropriate action may include corrective measures, policy review, training, operational changes, escalation, or closure of the incident record.
The amount of time required to review an incident will vary depending on its complexity, severity, and the information available.
Confidentiality, Good-Faith Reporting & Non-Retaliation
Prime Allied Workforce takes incident reports seriously and handles reported information with appropriate discretion.
Information may be shared with individuals who have a legitimate business, legal, investigative, safety, or compliance need to know.
PAW prohibits retaliation against individuals who make a good-faith report, participate in an investigation, or raise a legitimate safety or compliance concern.
Submitting knowingly false, malicious, or intentionally misleading information may itself violate PAW policy.
Can I Report Anonymously?
Where permitted by the reporting process, individuals may choose to submit certain reports anonymously.
Please understand that anonymity can sometimes limit PAW's ability to obtain additional information, verify facts, provide status updates, or fully investigate a matter.
Providing contact information does not prevent PAW from handling the matter discreetly.

Important Reporting Guidelines
Reports should be made in good faith and should contain information the reporter reasonably believes to be accurate.
Do not include unnecessary sensitive personal information, medical information, passwords, financial account information, or other restricted information unless specifically requested or necessary to explain the incident.
Supporting documentation should be relevant to the reported event.
PAW may preserve incident records in accordance with applicable document-retention, legal, compliance, insurance, safety, and business requirements.
